The three-year plan outlines the ambitions for how adult health and social care services will continue to integrate and how Angus Health and Social Care Partnership (AHSCP) will encourage people to improve their health and wellbeing.

The priorities have been developed in collaboration with people who use health and social care services, people who may use services in the future and partners in Angus Council, NHS Tayside and third and independent sectors. 

The priorities for the next three years in Angus are:

• Improving health, wellbeing and independency
• Supporting care needs at home
• Integrate and enhance primary care and community responses
• Integrate pathways with acute and specialist providers for priorities in care

Vicky Irons, chief officer AHSCP, said: “The plan reflects the many conversations we have had with the people of Angus and our professional colleagues across all sectors and services about what health and social care integration means to them and the services that they value highly.

“A lot of good work is already taking place across our communities but we know more needs to be done if we want to see further improvement to the health and wellbeing of the people of Angus and to reduce the health inequalities that exist in Angus. This plan is ambitious but we are also realistic about what can be achieved with the resources available to us.”
